Observation

During onsite inspection, Licensor reviewed the onsite facility site records and did not observe a State Fire Marshall form completed and requested to review the form. Provider Shukri Jama was unable to produce the completed Fire Marshall form when requested. (2) During onsite inspection, Licensor observed a red plastic gas container near a lawn mower accessible in the licensed outdoor area.
